Addendum Scenarios

Let’s review a few common situations where an addendum is needed to update a patient note:

Note: In order to add an addendum to a discharged case, you'll need to follow this process to reopen the case.

Scenario #1 - Typo

  • I made a typo while documenting

For this situation, your best course of action would be to:

  1. Add an addendum to the note.
  2. Make the necessary updates to the note.
  3. Then, Finalize & Rebill, Finalize (if you don’t have a billing integration), or Forward for a co-signature.

Scenario #2 - Incorrect Note Type, Incorrect Patient, Duplicate Note or I need to Delete a finalized note.

  • I finalized the wrong note type
  • I finalized a note for the wrong patient
  • I accidentally created a duplicate note
  • I want to delete a finalized note

For each of these situations, your best course of action would be to:

  1. Add an addendum to the note.
  2. Then select the No Charges This Visit checkbox in the billing section of the note. This checkbox is located on the Objective Tab in Daily Notes and on Billing Tab for evaluative notes (Initial Evaluations, Progress Notes, Discharge Summary)                                                      
  3. Then, Finalize (if you don’t have a billing integration), or Forward for a co-signature.
  4. If you have a billing solution, you’ll need to delete the note made in error from your system.

If you’ve finalized the wrong note type, select and complete the correct note type.

If you’ve finalized a note from the wrong patient, navigate to the correct patient’s chart, and complete the note.

Scenario #3 - Update Referring Physician or Update Patient's Authorization, Insurance Information, Demographic Information

  • I need to update the patient’s Referring Physician
  • I need to add/update the patient’s authorization
  • I need to update the patient’s insurance information
  • I need to update demographic information (i.e. DOB)

For each of these situations, your best course of action would be to:

  1. First, update the information in the patient’s case.
  2. Add an addendum to the note.
  3. Then, Finalize & Rebill, Finalize (if you don’t have a billing integration), or Forward for a co-signature.

Note: If you are changing which insurance is listed on the case as Primary, it is recommended that you perform a Quick Discharge and start a new case using a Placeholder IE.

Scenario 4 - Notes were not finalized in chronological order

  • The visit count is off because the notes were not finalized in chronological order. For example, the note for 5/7/2021 Date of Service shows visit 12 but 5/12/2021 Date of Service shows visit 11 because the note for 5/12/21 was finalized before the note for visit 5/7/2021.
  1. Add an addendum to the note with the incorrect visit count.
  2. Change the visit number to reflect the correct count on the note.
  3. Then, Finalize & Rebill, Finalize (if you don’t have a billing integration), or Forward for a co-signature.
