Add Patient: Patient Info

You can use the Add Patient link in the Patient Manager tab to add a patient to WebPT EMR.

Then add the patient’s demographic information into the fields shown below.

Ensure you complete all required fields: 

  • Status, Defaults to Active
  • First Name
  • Last Name
  • Sex
  • Date of Birth: The system automatically identifies patients under 18 and provides the option to use a Habilitative Profile. 
    • In some situations, you may need to determine how the patient will be treated and how you will use documentation notes in the patient record. For example: A 6yr old patient who needs speech therapy or a 17yr old who needs PT for an ACL tear. It’s more likely you would treat the 17 yr old as an adult rather than a true pediatric patient. Selecting Use Habilitative Profile? Will set up SOAP notes with a pediatric medical history and F.O.R. details.  
      Note: If the front office is not comfortable with making this assessment, the provider can complete it later. 
  • Language Preference, Defaults to English
  • Is Guarantor, Defaults to Yes for adults and No for minors
  • Custody Status (when using the Habilitative Profile)
  • Patient Info Notes:

    When using the Habilitative profile, the following changes are made to documentation:

    If the patient is a minor, the Employer Name field will be changed to Parent/Guardian.

    If the patient is a minor, the Is Guarantor cannot be changed.

    Billing Integrations

    Therabill

    The following fields will populate information in Therabill:

    • First Name
    • Last Name
    • Date of Birth
    • Gender

    If the insurance requires a middle initial to process claims, place it at the end of the First Name field. 

    Note: If a patient has a suffix (Jr., III), and it's on their insurance card, then the suffix needs to be added to the Last Name field.

    WebPT Billing

    The following fields will populate information in WebPT Billing:

    • First Name
    • Last Name
    • Date of Birth
    • Gender

    Note: If a patient has a suffix (Jr., III), and it's on their insurance card, then the suffix needs to be added to the Last Name field.

    Keet

    The following fields will populate information in Keet:

    • Patient Name
    • Date of Birth
    • Gender (sex)
